SDC3260 Blown Mosfet

We have run into an issue with what looks like a MOSFET for Motor 2 blowing on and SDC3260 (I can't attach a picture because the size limit is too small). This happened with no motors connected, and as soon as Motor 1 and 2 + and the P terminal. A loud pop was hears, then everything seemed to be working (could flash scripts), but then noticed issues when trying to drive motor two. Does anyone have any insight as to why the MOSFET blew? It happened with nothing connected to the motor terminals.
